Medical Records

To request records we must have a record release completed or a note stating patient name, date if birth and where records are to be sent, it must be signed and dated by the parent unless the patient is 18+ years old and they must sign.

The fee is $37.00 for the 1st 20 pages and $0.15 per page thereafter. You will be pre-billed before the records are sent out. Medical record requests are handled by PhotoStat once a week.

Form Completion

There is a fee of $25.00+ for form completion or letters.

Financial

Payment is expected at the time of service. We collect co-pay, deductible and/or co-insurance. Please be prepared to pay before you see the doctor. If you are unable to take care of your financial obligation you will need to reschedule your visit. We accept cash, check, MasterCard, Visa, Discover and American Express.

Bad Weather

If the Dallas Independent School District is closed due to bad weather our office will be closed. You will be contacted the first day our clinic re-opens to reschedule your/your child's appointment. If we open the office at a later time the information will be on our after hours message by 7:00 am. Just call our main number.

Cancellations

We require 24 notice if you are unable to keep your appointment. If you do not cancel your new patient appointment at least 24 hours in advance, we may not permit you to reschedule. If you cancel a follow up appointment with less than 24 hour notice or do not show we charge $150.00 that is not billable to your insurance carrier.

Prescriptions

If you need a refill on your non-controlled prescription, call your pharmacy and have them fax the request to our office. Controlled drugs may be faxed, emailed or a message left on the pharmacy line. We required 24-48 hours to process refill requests. There is a fee of $15.00 for controlled medication requests and $25.00 if it is to be picked up the same day as requested.

Phone calls and Questions

We employ specifically trained staff who will help you by telephone in an effort to increase availability and provide medical service.

  1. To contact us for questions regarding your child's neurological problems please call (972) 566-8600 and follow the prompts to the nurse line.
  2. You must leave a detailed message and your child's chart will be pulled.
  3. The staff will review your questions with the doctor and call you back.

Please do not call the operator or receptionist as they are unable to put your call through, you must leave a message on the nurse line. As our nurses often work until 6 or 7 pm your call may be returned late in the day or early evening.
